This may be from an HD mod because the concept art, promotional art, and even the game itself is conflicted about whether or not she has a piercing, a gem, or just bare.
As someone who played that game on a PS2 emulator on PC, I can confirm that at least ingame, she has a gem. And at the very least, that screenshot might've come from that emulator, it's simply using the emulator's capabilities to display it at a higher resolution than native.
